OFFICIAL TRANSCRIPTS
FORMER STUDENTS only need to complete the Former Student Transcript Request Form to obtain official academic records once they have graduated or withdrew from Seneca Valley.
CURRENT STUDENTS need to observe the following procedures when processing transcript requests. Most schools will require an official transcript of high school records to complete a college application. An official transcript package consists of the student's official transcript, a copy of the Seneca Valley Profile, and a copy of the Academically Challenging courses that were available to the student throughout high school. After the first semester, a copy of the student's report card will also be included in the official transcript package.
Students have the option of picking up their official transcript package or having it mailed directly from the guidance office. If a student wishes to deliver the records to their perspective school or institution themselves, the records will be prepared in a sealed envelope with an official signature and date. If the student requests to have the official transcript package mailed from the guidance office, the student must provide an addressed envelope and proper postage.
PROCESS and REQUIRED FORMS
If you are requesting a transcript for the first time you will need to complete and submit a Transcript Release Authorization form and a Student Information Sheet. Transcript requests will not be processed without first completing these forms. Once these two forms are on file in the guidance office, a Transcript Request Form will need to be completed for each transcript you request.

Transcript Release Authorization - this form must be completed once - if you are not 18 years of age you need your parent/guardian to sign - and returned to the guidance office.

Transcript Request Form – you will need to complete this form for each transcript requested (official or unofficial) and submit to the guidance office when requesting your official academic record.
All forms are available in the guidance office or can be downloaded from this site.
Before submitting your request for official records, please read your application carefully to determine which items are required. If your application has a supplemental form (ie: secondary school report) it must be submitted with your transcript request in order to process all required information.
TRANSCRIPT REQUEST FORM
When completing a Transcript Request form, please be aware of the following:

A transcript request form is required for each transcript requested (if you ask for 3 transcripts, you must complete 3 forms.)

You must specify the name of the school, scholarship committee, or person/organization receiving the transcript.

You must provide an addressed envelope with proper postage if you want your transcript mailed from the guidance office.

You must sign and date each request.

The guidance office may require up to seven (7) school days to process your transcript request - please plan accordingly.

Please check the appropriate items on the form - if you are not sure, ask one of the guidance secretaries for assistance before submitting your request.

Students are permitted one (1) transcript per school/scholarship. Additional transcript requests for the same school/scholarship will require a $1.00 fee (this does not apply to requests from schools/institutions requiring mid-term grades or updated information to assist with admission determination.)
